CBP Form 5106 (Importer Identity Form) is required by U.S. Customs and Border Protection (CBP) to register an importer before a shipment can clear into the United States. You can view the form at cbp.gov.
What is CBP Form 5106?
Form 5106 allows CBP to identify and register the importer or consignee in their system. If you're importing goods into the U.S., CBP must have your details on file before they can process the shipment.
When Form 5106 is required
You'll be asked to complete Form 5106 if:
You are importing into the U.S. for the first time, or
You are not yet registered with CBP, or
Your importer details are missing or incomplete
Carriers such as FedEx, DHL, or UPS typically request this form when needed. It is most commonly required when:
The shipment value exceeds $2,500, or
You are a new importer, or
Your details are not recognised by CBP
