CBP Form 5106 (Importer Identity Form) is required by U.S. Customs and Border Protection (CBP) to register an importer before a shipment can clear into the United States. You can view the form at cbp.gov.

What is CBP Form 5106?

Form 5106 allows CBP to identify and register the importer or consignee in their system. If you're importing goods into the U.S., CBP must have your details on file before they can process the shipment.

When Form 5106 is required

You'll be asked to complete Form 5106 if:

  • You are importing into the U.S. for the first time, or

  • You are not yet registered with CBP, or

  • Your importer details are missing or incomplete

Carriers such as FedEx, DHL, or UPS typically request this form when needed. It is most commonly required when:

  • The shipment value exceeds $2,500, or

  • You are a new importer, or

  • Your details are not recognised by CBP

Information collected

The form includes:

  • Legal name (business or individual)

  • Address

  • Tax ID (EIN for businesses, or SSN for individuals)

  • Contact details

Why CBP requires this

This information allows CBP to:

  • Create or update your Importer of Record (IOR) profile

  • Process your shipment through customs

  • Ensure compliance with U.S. import regulations

  • Maintain accurate trade records

Consequences of not submitting

Without Form 5106:

  • Your shipment cannot clear U.S. customs

  • Delivery will be delayed or held indefinitely
